"Have you ever been taken to the State Fair?" is passive.
"The team won the state championship." is active.
To differentiate is really easy! Passive voice is constructed by using "to be" and past participle (e.g. bought, cost, made, done, etc.) and put enfasis on the action, not on the person who did/does it.
